The HMG model has been implemented throughout the U.S. and in half of California’s counties. First 5 Contra Costa, the Contra Costa Crisis Center, and La Clinica de la Raza came together to develop a pilot to integrate ACE screening and response workflow utilizing the existing HMG model.
