Great content but resources missing

I would love to give this audiobook five stars. The content and advice is very, very good. But, there are forms and questionnaires to complete as part of the self-evaluation process mentioned throughout that are not accessible for audiobook listeners. Most books like this that have additional resources necessary to get the most out of them provide those resources on the book's page on Audible. This one doesn't.
